The interim online pre-registration form is a short-term solution that helps identify eligible individuals in East Toronto who are interested in receiving the vaccine
(March 3, 2021) Toronto, ON 鈥� East Toronto Health Partners (ETHP), the Ontario Health Team (OHT) serving East Toronto, has launched an interim online pre-registration form for COVID-19 vaccine appointments for eligible priority populations in East Toronto.
The form is a short-term solution that helps identify individuals in East Toronto who are included in Phase One of the Ontario government鈥檚 vaccine implementation plan and would like to express their interest in receiving the vaccine. The information captured in the form will be transitioned to the centralized booking system that the province is developing and is expected to launch on March 15.
The interim online pre-registration form is accessible through and Michael 九游体育鈥檚 (MGH) website. Eligible individuals who complete the form will be contacted to book an appointment at a local COVID-19 Immunization Clinic when vaccine supply is available for the month of March.
ETHP will help facilitate support for individuals who indicate that they need help getting to an immunization clinic. Caregivers may complete the pre-registration form on an eligible individual鈥檚 behalf.

Completion of the interim online pre-registration form indicates an individual has expressed interest in receiving the COVID-19 vaccine. It is not an appointment confirmation.
More than 2,000 individuals in the eligible priority populations in East Toronto completed ETHP鈥檚 interim online pre-registration form on March 2 when it was launched.
The interim online pre-registration form helps facilitate , which includes deploying mobile vaccination teams and operating COVID-19 Immunization Clinics in East Toronto. Developed in consultation with the Ministry of Health and Toronto Public Health (TPH), the Plan helps ensure all East Toronto residents have low-barrier access to the COVID-19 vaccine as soon as it is available to them.
The Plan complements TPH鈥檚 and other hospitals鈥� and OHTs鈥� vaccination plans across the region which may vary depending on the supply of vaccines, the specific priority populations they are vaccinating and characteristics of their communities and networks.

We will continue working closely with our health system partners to ensure COVID-19 vaccines reach the arms of our priority populations in a manner that is as safe and convenient for them as possible.鈥�
